Our required enrollment application is linked above. In addition to this application, we request the following documents:
Birth certificate
Current immunizations (including Tdap for 7-12th graders)
Proof of residency (utility bill or rental agreement)
Recent grades/transcript
Current IEP or 504 Plan (if applicable)

It is recommended your child remain enrolled at their current school until you receive confirmation of enrollment from us.
